This survey will take about 15-20 minutes to fill out. You will not be able to save your progress and return, but you can spend as much time as you need on the page without losing data, unless the page reloads.

There is a lot of food options, some are “diet” foods, and some are “fun” foods. You’re allowed everything on the list, so if you might like it as an option in the future, please pick it. The more the better. Don’t not pick something because you think you aren’t allowed to have It. Let me be the judge of that. If you pick 10 foods on this list your diet is going to be very boring, and more than likely, you will be burnt out in a week. I want this to be a long-term diet, not chicken and rice every meal. I will try and incorporate some of the foods you enjoy, making the process a little easier!
